Although the source of this video is American, the first 15 minutes could be describing cycling policies of Ontario municipalities and why they are failing.

From ~5:30 mins into the video, images should be recognizable to those who live in cities that have built dedicated cyclist infrastructure.  The last 15 minutes address the solution, one which we have had available for decades but have never taken seriously nor assigned any priority.

(It is acknowledged that the US legal and historical references are different. In Ontario a cyclist is a driver of a vehicle and as such there is no "far to the right" requirement in our traffic law. And though provincial laws in Canada have often reflected a bias in favour of motorists at the expense of cyclists, there's not much evidence of an organized "motordom" influence in Ontario. )
