Reading through all the suggested amendments to the adverse results reporting and corrective action,
what strikes me is there is an assumption of literacy, intelligence, and experience. It's been a real eye opener for me to see and experience persons struggling with the tasks they are responsible for when they are really not up for it, or are juggling other responsibilities in the course of their employment.
Providing graphics and or flow charts that show a step-by-step process of submitting an adverse result and corrective action could be very helpful, especially for someone dealing with this for the first time.
